REGISTRATION FORM

At Steeplechase Veterinary Hospital we record information about our owners and their pets into our management software so that we can effectively and efficiently provide the service you deserve! To make the initial check-in process faster for you, we recommend that new clients use our on-line registration form, which can be filled out in the comfort of your own home or office and then sent to us by entering “submit.” In addition we encourage you to transmit your pet’s recent Veterinary records or provide us with the contact information so we may retrieve them for you.

BOARDING DROP-OFF AND CONSENT FORM

For those who wish to complete our Boarding Drop-off form in advance, Click Here, print, fill out, and either email or bring it to the hospital with you.  If you are a new client, we request that you also complete a New Client Registration Form so we may serve you better.

SURGICAL INFORMATION PACKET

This information should help you understand what will happen from 9:00 pm the night before your pet's surgery until you take your pet home from our hospital.  Pre-anesthesic blood testing is explained.  And information is provided on procedures, pain management, and potential risks.  We hope this will help make surgery day as easy and stress-free as possible for you.  If you need further information in advance, please telephone us at 678-208-2200.

RECURRING PAYMENT PLAN 

Checking/Savings Account

Our bank will contact your bank to automatically deduct your Wellness Plan payment from your checking or savings account with powerful security protection – more secure than paper checks or mailed payments

The Recurring Payment Plan will help you in several ways:

  • It’s convenient (saving you time and postage)
  • Your payment is always on time (even if you’re out of town), eliminating late charges
  • It’s easy to sign up.  Fill out the Recurring Payment Plan Form and return it to Steeplechase Veterinary Hospital

Here’s how the Recurring Payment Plan works:

You authorize regularly scheduled charges to your checking/savings account. You will be charged each billing period the total amount due for that period. A receipt of payment will be emailed to you and will appear on your statement.
